After Hours Emergencies

If you are experiencing a medical emergency, our physicians recommend that you Dial 911, or go to the nearest hospital emergency room. However, if you need after hours emergency service that cannot wait until lthe next day, call (801) 546-9441, and our on-call provider will be paged.

Medical Records

A medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting that your records be sent to another provider we ask that you supply us with their name, address, and phone number.
